The reasons seem reasonably clear to me.
MYL didn't raise the required funds in the first attempt. And it never even attempted to re-list the 2nd time. Instead, it just tried to have the 2Y removal from quotation rule waived by the ASX (but was unsuccessful for reasons it never disclosed). Even if the ASX had extended the deadline by several months, John Lamb would obviously have been unable to meet the new deadline anyway (as demonstrated by his decision last week to delay the IPO again)
