Hi there Q, so filling in the form, haven't done one before so a bit unsure, so is it as follows:
1/ Fill in name and details
2/ Apoint the chairman to vote on your behalf by x the box
3/ vote for on all but conversion
This is the bit of the form that unsure, all the member 1, 2, 3 and directors and secretaries bit at the bottom of the form?
Then what is the fax #
