Manufacturers price drugs by agreement, and after negotiation, with the buyers, who are usually insurers, or government bodies.
A single price is agreed, and that's the price you pay. Charging varying prices according to the medical indication for which the drug is used ('indication-based pricing'), has been suggested by a few academics and health economists, but isn't actually used anywhere at all.
I don't think there's any chance whatsoever of PPS being priced at anything like the numbers that you are suggesting are possible, although this would be nice for shareholders.
