Finally got my response from Computershare regarding options conversion and, as promised, I'm reporting back.
If anyone else is interested, looks like there's no option conversion form on the site (that I could find) so you need to contact them directly to get it emailed to you (or sent by post, I suppose).
I've got mine and I'm all set for when HAV declares the 5c dividend.
