Some doctors don't care since, after all, they are not paying the bill. From my experience working in a hospital, some time ago, it wasn't difficult for some people to obtain a disability certificate from certain medicos. That even extended to 'monetary' disability, when it was very obvious that not all income was being disclosed, especially in relation to the self employed.
