Well, while I'm not ready to convert yet, curiosity got the better of me & I logged on to Automic just to see if the form was there. Although searching thoroughly through website I could find no facility to request/download the form. Oddly enough, my PTXOC holding cannot be selected & appears as a subset of my PTX holding which is selectable.
I'd suggest anyone who has trouble to ring the Automic Registry direct as they will at least advise you requirements.
