....Jack Toby sorted it all out....I received my form, signed my form, emailed my form back ....all acknowledged...in a very small time frame

....only cost me 10 minutes of my life......

....silly, incompetent Computershare.....why did I ever bother???? four emails ( unanswered), three phone calls...nothing resolved

PS..... I shouldn't really pass this on....but the computershare third person who I dealt with was so painfully inadequate, surly and rude ..."you need to write to us and request the form, we just can't send you a form"....I congratulated her that she actually had a job....a first time ever for me ...but I just was sooooooooooo frustrated with someone answering the phone and pretending they were "customer service"
