Here's what I did, which resulted in a quick turnaround. FYI I'm an individual investor, not a trust or company or SMSF or anything special like that:I didn't get a response / receipt / etc., but the options were converted to shares within a week I believe.
- Printed out the form from the website
- Filled out the form and signed it where it says "Individual / Security Holder 1"
- Scanned the form back in to my computer
- Made the payment via bank transfer in online banking
- Took a screenshot of the bank transfer receipt
- Sent an email to the MAY address listed on the form, attaching the scanned in form and the screenshot
