I've been trying to think of simple ways to explain the concepts of efficacy and futility - I suspect I've been around clinical trials for so long that I assume some things are self evident, so I apologise for that!
One way of looking at it is to see that overwhelming efficacy and futility are two sides of the same coin. And bear in mind all of this is about probabilities.
Say for instance the treatment group is outperforming the control group. The question is, is it doing so much better that there is no chance of it failing if the trial continues. That is overwhelming efficacy.
Suppose that on the other hand the treatment group is underperforming the control group. The question is, is it doing so much worse that there is no chance of it succeeding if the trial continues. That is futility.
