    one good thing is that repeat trials for this should be relatively inexpensive, which was the point of the app, cost friendliness.

    the acoustics should be tested comprehensively in typical dreadful ED conditions. It is sometimes unbelievably loud in there. Imagine some demented old biddy demanding to go home, shouting for all to be heard, in the next cublicle, a resuscitation, in the next a monitor bleeping incessantly because an oximeter probe is off, the nurse off attending to the resus, in the corridor all manner of traffic, trolleys, wheelchairs, relatives, ambient chatter about patients, the police bring in a cuffed tweaker, shouting nonsense. As these are real world conditions, this app needs to work there consistently at a high level of confidence.

    This isn't a pleasant way to find out there are environmental acoustic issues. But now that it is identified, perhaps even simple tech might help, eg the progenitor of the modern stethoscope itself, a rolled cone of cardboard or disposable funnel over the patient's mouth (can't imagine your average rug rat agreeing to it but anyway, maybe worth a look to see if it excludes much of the noise from the signal)
