How about 'given the fact the noise in ERs and large hospitals is out of your control, has thought been given to administering in large medical practices instead (i.e. Gp clinics?) private room, less noise, less time pressure, less likely patient is critical, calmer doctor, more familiarity with patient, more time to adhere to protocol

Oh, and more analogous to the eventual telehealth environment (quiet room for phone consult rather than busy hospital)
