    B-cyclodextrin models improved BBB penetration for doxorubicin in an in vitro model by a factor of 2 and 3.7 (1). Doxorubicin has a molecular weight of 543.5 (which is greater than 400 Da) and 6 and 12 donor/receptor hydrogens. This means that on its own, doxorubicin wouldn't be able to cross the BBB, but combining it with this advanced delivery technology helped to transport it. It's kind of like trying to sneak through a tight hole in a fence and someone comes along and pulls it open a little more for you.

    The City of Hope researchers also combined Bisantrene with B-cyclodextrin in their complex in vivo studies (more complex than your basic xenograft). Bisantrene in combination with B-cyclodextrins has shown positive data in patient derived xenograft (PDX; taken from relapsed human patient and put into mouse) AML (top) and immunocompetent (BMT) breast cancer mice (bottom). I've included brequinar to demonstrate how much better Bisantrene performed in these models. In both figures, percent survival on is on the side and days of survival is on the bottom.

    Something which is also interesting for me is that the City of Hope also used Bisantrene in Glioblastoma (GBM) cells, which it outperformed Brequinar again (below; the green line being closer to 0 means that it had more effect).

    Glioblastoma is really nasty with a very low overall median survival. One of the major challenges is delivery of chemotherapeutics to the tumor location accross the BBB.

    It wouldn't make sense to me why City of Hope would review Glioblastoma if they didn't think Bisantrene could influence this cancer type in vivo. There are plenty of other cancer types that have a similar FTO expression pattern to AML.

    I don't know enough about advanced drug delivery technology to really comment about the likelihood of it being successful or if there are options available. It's interesting to think about nonetheless!
