My thinking for these conservative numbers is that 1 patient has been treated and assessed.
Reasoning is that you want to observe (and understand) how the first patient responds. As an analogy in engineering you test a first article (first in production) before moving to production and testing of following articles in a production series. It's a cautious/methodical approach.
