I would have thought this approach applies to cohort 1 and cohort 2 (and is in fact the exact reason for the two cohorts), but not the patients within each cohort. Otherwise there is effectively six cohorts rather than two. To me it would make most sense to dose all patients in cohort 1 as quickly as possible and evaluate how they respond as a group before making the decision regarding cohort 2.
Perhaps Dr. T can clarify that.
