I believe looking back over previous posts, Rick has said that the email to send your details to/proxy form or information of how to fill out your proxy form will be sent to you once you email your interest is:

Believe if you send your name, hot copper name and the number of shares you hold (which you have indicated in your post) to that email, you will receive a response with what to do.
