No, if that were the case, the buyer of the shares from the borrower would not own the shares.
What I think happens is the lender gets an IOU from the borrower, stating exactly how many shares are loaned out, together with a contract stating that the borrower will pay all dividends to the lender as if the lender still had the shares.
The shares have literally been loaned out from the lender to the borrower. If the borrower sells the loaned shares, whoever buys it from him in the market has clear title to those shares.
