Standard of care is debridement, dressings almost daily, more debridement, more dressings - all along with antibiotics if infection continues. Plus the occasional angiographic intervention to improve vascularity if it’s deemed possible or likely to improve the situation.
Never heard of the cast bit - seems counter productive as the wound would just fester.
If Synpath trials are as successful as we think / hope then it will be adopted universally and will be way more cost effective and soooooo much better for the patients
