I reckon targeted ultrasound guided injections are the way to go. It is common practice now in minimal intervention. I am convinced that MTX as a scaffold helps with tissue regeneration and supports the increased blood supply necessary for healing. Rotator Cuff surgery in the elderly has a high failure rate and patients are reluctant to go back for more open surgery. In the UK NHS you can imagine the benefit of an injection as distinct from waiting for 2 years for a cuff repair by some registrar who might be on a learning curve. Perhaps in the future if the results are good the ultrasound guided injection is the treatment of first choice in partial tears. I am convinced that MTX will be used extensively to encourage blood supply healing in such areas as tendon repair, nerve repair and in the fractures in the frail and elderly whose blood supply is a limiting factor. I am beginning to sound like DW, perhaps i can get a jacket for Christmas.
