Look at how long the process to verify a fail safe life saving device has taken MBO...and then the process of verification as to suitability as such a device at depth.
The device must operate at depth and survive water intrusion and then be capable of transmitting a (recovery) signal on the surface?
Plenty of scope for things not to work...the process continues.................
