really good point and an issue with unblinded controlled trials.. was an issue with VISION as well. if patients got randomised to SoC they could just bail and pay for treatment elsewhere - going on the trial means 50pct chance of getting the treatment for free.
unfortunately theres no real way to aboid this with radionuclide therapy - theres even mobile apps that can detect emissions now so cant even do sham infusions for blinding...
