It says on the form that whatever amount you pay via bpay is the amount of shares you have applied for. So for example if I were keen on getting 50,000 shares in total, and my entitlement is say 20,000 then I just do a bpay payment for 50,000 @ 0.975 cents = $4,875.00. No need to return the form as your bpay number relates to you only and they will know that by paying them $4875.00, you have applied for 50,000 shares + attaching options. Done this heaps before with other stocks and never had any issues.
Though I am also waiting until about 8th August to find out first what happens with debt crisis in USA. During GFC stocks like MNM crashed 70 - 90% in price so might yet buy on market if that were too happen.
