Maybe. But the Board should resign apart from new directors who have come onto the Board after the incident occurred.
I would be interested to read the internal audit reports and the external audit reports as to whether this was raised as an issue and what management said they would do about it. From these reports you can really get to understand the deep seated culture of a company.
For full disclosure it took me more than two years to get a refund paid for flights that Qantas cancelled very early in the Covid pandemic. Unlike a US hotel chain that made a 100% refund without us having to even apply. When I finally go to the US I am staying at that hotel no matter what. They have my loyalty and I am not even a customer yet.
