Usual reasons are:
1. Order reaches expiry date. With Commsec it used to be 25 trading days from memory. Don't know whether that is still the case.
2. Is outside the allowable price. If the current price moves too far from the order, the order purges.
3. Change of status of the stock. e.g if a stock goes ex-div, prior orders are purged.
There may be other reasons. I think there's a list on the ASX site somewhere.
