Questions about Entitlement Offer

I have a retail entitlement offer for BTH shares and had a few questions about how they work.

1. I use commsec as my broker, if I fill in the form here and make payment, do I have to do anything else to get them to show up in my commsec portfolio?

2. There is an option here to apply for new shares as well as the entitlement shares. How do these new shares usually work? If I choose to apply for more shares, are these likely to be accepted? Or are they typically not accepted? If I pay by BPay, and they are not accepted, how do they send the payment back?
