Great list, I would just give Sue a bit more than 3 months to do anything.
"Sue, how do you envisage your leadership will differ from past management, and what can we expect to see within the next 3 months?"
I'd also suggest prioritising these questions or grouping some of the issues together as a general "here are some examples of targets missed/false communications" because there's no way they will get through all of these. You want to make sure the important questions are asked rather than getting lost in the details.
Other than that, if you can get even a few of these answered it would be great.
