One question that should have been asked but wasnt:

After 2 years of dispappointing production following general predictions. The Managing director this year made a series of specific targets for three quarters which almost certainly will not be met. At what stage will the Managing Director admit that the forecasts were mis-informed and that the qualifications of the Managing Director and the requirements of the position are not aligned?

Unless the situation turns around soon the board must insist on replacing the MD with one responsible to the board and shareholders for meeting specific goals.
