In answer to your first question, the aspect that causes most variation is if the drug actually reaches the target tissue. You could have a cardioprotective drug that works in cells, but if the drug doesn’t reach the heart at an high enough concentration then it won’t work in a human.
The answer to your second question is if you do the experiment correctly the probability it is a coincident should be near zero.
