Your experience suggests that this hypothetical scenario may have happened.
Automatic ran the batch job to create the options exercise form and instructions in two subgroups.
For the first subgroup, they created the exercise forms but not the instructions. Not a serious problem; what to do is explained on the exercise form anyway.
For the second subgroup, they created the instructions but not the exercise forms. This is a very serious and tricky problem, because to rerun it involves using the same selection criteria and the shareholding as at the original run date, not today's holding.
