I exercised my $0.75 bonus options last year. Never received any mail or email of a 'Notice of Exercise of Options' form for the $1.25 'piggies'. Logged onto Automic to see if the form was there - also non-existent, much like the hassle with getting my bonus options exercise form.
After emailing Automic and requesting the form, it was "generated today" and now appears in the portal. Interestingly, it has the old RAC logo...
Just in case anyone is awaiting this from, seems you'll have to ask for it...
