There are two problems, but they may be linked.
1 People whose exercise form simply hasn't been created. Cause is unknown.
2. Peoole who didn't receive the second email.
If the selection criterion for the second email was "exercise form exists", the underlying problem remains.
AFAIK nobody has reported "received second email but the link didn't work"
