It is usually best to go off the body of the candle for the top of the cup, but i have seen both cases where it goes off wick.
You can have many patterns inside the cup+handle, however the bigger the timeframe your using, the more-true the breakout will be.
ie. the weekly cup and handle is more promising than a cup and handle on the daily, daily is more promising than cup and handle on the hour etc etc
