e have a drug. Its not only highly efficacious but it has HUGE global applicability. Like I'm talking not just a decent sized runway, I'm taking a few giant runways all patched together and magnified a couple of times over.

An Inflammation Primer )....but tonight we take a slightly different angle. Yep time for a smooth red, perhaps a morning coffee...even try one of those fancy gin and tonics that's becoming all the rage...but drink in moderation of course.
CAN I BE SO BRASH?
Can I start out by simply suggesting that the underlying reason for joint damage has one main culprit.
Is it age?
Is it how active you are or aren't?
Maybe it is genetics?
Well maybe all of the above but the one single major culprit is....
Inflammation.
Lets explore.
I'm certainly not suggesting we need to squash all forms of inflammation out totally. This would be the same as suggesting a total NGF blocker like
Tanezumab has only positive merits. In other words, we do need
some inflammation. A quantity of inflammation keeps us in check, it keeps us safe, it elicits the right reactions, the right bio chemical processes in order to safely alert and enable the body to address the underlying issues causing it.
Problems happen when there is too much inflammation. Problems will also happen when you attempt to block inflammation all together, immunosuppressants, we do not want this, we want
down regulation.
The best analogy I can come up with is that you work at a fire station and the alarms go off. You've planned for this, you are ready, you've been through the drills so many times you don't have to think, you fly into auto pilot mode and you are ready to attack the fire. BUT......what if those alarms never go off....what if you go deaf and crazy and all you can do is panic and try (and fail) to put off those alarms and you effectively get nothing done because all you do is try and scramble for the kill switch.
Chronic inflammation (the never ending fire alarms) cause a lot of damage, we focus on OA...but there are many many types of conditions and I get my 6 monthly reminder from
@Ave Maria about the many types of inflammatory conditions that abound.
THE FINE DROP
Some amount of inflammation is good, it creates the response required for the body to attempt to adequately repair the injured tissue. It does this in several amazing ways we all take for granted. It allows vessels to dilate...this in turns allows for more blood flow to the site supplying nutrients and important cells such as macrophages to mop up, clean up, disinfect.
Within a joint, inflammation also occurs in order to remove injured or foreign material...think bits/fragments of cartilage for instance.
But like most things with the body, the process is so much more complex...another example is the actual fluid makeup that gets changed. And there is the perfect lead in to our friend HA. HA, Hyaluronic fluid, is a marvellous drop. Here is my fine drop story:
MOZZ STORY TIME
I don't mind the odd red wine...I once was asked by a fellow share options conference attendee for some practical share option help. I spent about 2 hours assisting him (which seemed like 10 mins to me as I love talking about options), explaining what I do and giving him my 5 current trades that I wanted to do. He was so happy I helped, that he asked me if I'm coming to next year's conference in the USA.
I simply stated that I take a pragmatic approach to these seminars each year, if I make profit enough on my US options to pay for the trip, I'll come. He then asked, well what if I shouted you?
Mate, I said if you are paying for my air fare next year, I'll be there for sure! Not really thinking he would go through with it, I waited a few weeks and he not only shouted my return trip...MEL to LAX, LAX to NYC, NYC to Miami and back...he upgraded me to Business Class. All the way...True Story. The boys at work had a field day saying it'll turn out to be a drug mule thing...they were also pretty impressed when it turned out to be legit and I came back in one piece to tell them about it!
What does this have to do with HA? What does this have to do with a good drop? Well my analogy was the wine they served in Business Class on the way over to LAX, It was a Californian Red Shiraz and it is the single smoothest Red Wine I have ever had.
Lets get back to our own good drop - HA.
This is the quality-likeness of Higher Weighted HA. I think of that above story every time I think of HA. This is the drop
we want.
BUT...when inflammation abounds look what happens to this very same fluid...the inflammatory processes actually changes the very constitution of the synovial fluid...it introduces excess fluids along with a higher amount of destructive enzymes and then the synovial fluid begins to lose viscosity. Another analogy...think what would happen to your precious car engine if you started corrupting the oil with grit...or something less viscus...eventual grinding of those moving parts? Isn't that the same as what would eventually happens to our joints? Wouldn't the ride be so much rougher?
HA is one heck of a super liquid...it holds
1000 times its own weight in water. It acts as a shock absorber taking on the daily load and biomechanical stress and loads. It keeps those valuable tissues of ours hydrated.
Here is a Mozz fact for you: When you are 50 years of age, you produce about half the HA as when you were a teenager.
Second fact, it is the Higher weighted flavour that has even more binding to water capability to the lower weighted version.
Keep that fluid as pure as can be.

KNOCK ON EFFECTS
Once you start getting a compromised synovium the pathway to destruction continues...
Inflammation penetrates and affects all parts of the joint, as an example:

The inflammation present in the synovium triggers changes in the peripheral nervous system, affecting the afferent processing of nociceptive signals from the joint and surrounding tissues".5

nociceptive is a fancy word for sensation of pain)
The articulate cartilage is compromised and cannot keep up with the repairs...the bone finally responds with a defence that causes further destruction and trauma. New bone is attempted to be laid down forming bone spurs and if left unaddressed...runaway inflammation as we know as arthritis will ultimately and totally destroy the joint.
HISTORICALLY
In the past it was assumed that OA was all mechanically driven. Its more recently that we know that inflammation plays a pivotal role and it is all encompassing of the joint and surrounding tissues and membranes. In fact in the case of obesity, yes there is an increase in mechanical load but did you know that the role in terms of additional pro-inflammatory cytokines (primarily produced from adipose tissue - subcutaneous fat) also boosts joint damage? In reverse, it is possible an obese person can mitigate at least some of the potential OA if their inflammatory cytokine counts are lower.
With the assistance of MRI we know understand that inflammation plays a role in all joint structures, we are talking inflammation of the subchondral bone that manifests into bone bruises, bone marrow lesions and cysts. In fact, "In the development of OA, subchondral bone becomes thickened, even before there is loss of articular cartilage". 2
Its nice to read about what happens, but how about seeing an example and comparison of the multifaceted destruction:
Inflammation truly plays a central role in the progression of OA. While the same inflammation is a central key component in the multiple repair processes of the body, it is runaway chronic inflammation that appears to be the one major targets of Pentosan via multiple actions and the addressing of the cascading nature of inflammatory cytokines. The down ramping of the alarms to once again bring back harmony and balance to
allow for the regenerative and repair processes to occur appears to be the required tipping point in many patient cases to date.
We aren't far from more quality data showing further evidence not only progression via biochemical biomarkers, but the
structural ones. This raw evidence will be very tasty in my view. To see, even at a micro-millimetre level, the differences in early joint space width, the differences in BMEL volume and grade....and guys, how about even just a few active drug patient's regression of BMELS??? This is the exciting stuff waiting for us...NOT in a few years time...in a matter of
weeks.
The world will finally know about the one naturally based drug that can potentially end this cycle of destruction caused by runaway inflammation in many future patients.
