And resulting from Malaysian Airlines continuing to fly over the Ukraine war zone to save time and money, despite multiple other airlines avoiding such routes for the safety of passengers and crew.

And, most importantly, resulting from Ukrainian authorities not closing the airspace over Donetsk, because they received revenue each time an airline crossed Ukrainian territory!
