Hi all,

I wanted to sell my small lot of shares using the sale facility before June 23rd. However I've lost the blue form!
Can anyone let me know where to get a copy, or better yet, email me one? I've spoken to Pacmag themselves but they have not been able to get me a copy of the form yet (have also spoken to the Share registry people).
