the 300mg trial dosage may not have blocked enough glucocorticoid receptors.
just from this line you can see the Author is making assumptions,, also aren't ACW utilising escalating doses??
Possibly the Author has jumped the gun ,, IMHO never say never until there is a very large lady warming up right of stage....
