I don't think anyone believes there haven't been serious management failings here. It's about whether the effects of these have been mitigated. We can all improve and learn from our mistakes.

You might ask for a copy of: "Fowler's Modern English Usage" plus a dictionary for Christmas. It's never too late. The 4th edition of the former was prefaced by:

"The previous revisions of H W Fowler’s magisterial work of 1926 — by Sir Ernest Gowers in 1965 and Robert Burchfield in 1996 — led to accusations that its editors were being too kind to ill-educated speakers of English who perverted its splendour by introducing barbarous usages".
