It is actually extremely rare for researchers to exactly pinpoint why medicines work the way they do. Outside of Benzodiazepine = Agonist for **a-,A receptors or Opiate = Agonist for Mu receptors, no one really knows 100% what is going on in regards to pharmacology.
The best examples are SSRIs. They STILL don't know the main mechanisms of action and why some people improve and some dont, yet have been prescribing them with success.
The most important element is safety and efficacy against signs and symptoms. NOT what is happening under a microscope. Trof has smashed both.
There is no way the MOA can be used as an excuse, if the drug is treating the condition and is safe long-term, otherwise it will call into question every other molecule/drug currently in circulation.
