Yeah it happens so often. They know they aren't reporting things correctly but they will blame a system or a process because it's nearly impossible to prove intent. In a world where trading systems have been digital for a very long time there is no excuse for any company (let alone ones with huge amount of resources) to incorrectly report these things in line with legitisation. The broker I had had a reasonable in house legal team and I was suprised how many lawyers were working full time internally at that organisation yet they constantly have breaches, and I think a lot of the time they make a financial decision on fixing underlying problems with culture and reporting vs the cost of defending and negotiating down the penalities.
