I'm not 100% on the lending process as whenever I've been involved in shorting, I've always been the borrower not the lender! It's never really been something I've cared about much. However I believe most of the time it would be an opt out system (or it would be a term of investing you're money with them, and if you don't like it you can go somewhere else).
From my understanding your broker (eg commsec etc) wouldn't actually lend out your shares, so you don't need to worry about that. Shares lent out would be with a custodian of some sort, eg you invest in a large fund. Many super funds lend, though some of the ethical ones choose not to. As an investor you don't have any say over this typically, and your only option is to choose a fund/custodian who won't lend.
Fees are typically what you would pay for a margin loan (a loan of shares is essentially the same as a loan of money)
