If a solution can be found - if it was specced too far wrong or the tailings material is simply far different to what was expected then it may not be possible to fix it. I've seen and heard of lots of stupid things happen over the years - one example was Woodside's Pluto LNG project in Dampier in W.A.- it has a massive flare stack that is probably 80 metres high and after the first one was built a vac-placement uni student came to the project for a few weeks and pointed to it one day and said "is that cyclone rated?" - had to be torn down and a new cyclone rated one designed and built.
