Yep I remember BT second hand plant and its disassemble and reassemly are points of serious issue.
Cyclopes - without going into detail there is much in what you say about timliness etc I think. Key is when they knew and that is being worked on.

Thin about it and CMR can sort of be a same case but in reverse. Deending on when they knew what.
Questions arise around when they knew what was wrong and when they disclosed it. If that can be answered ok the questions then move to why didnt they know - why was it a shock to them. Was it incompetence or negligence.
If incompetence then it is negligence coz from what I have read a diirector is expected to be competent and if he isnt he should remove hims;ef from the positon
