As requested this thread will host the information relating to the survey for BLT shareholders.
As discussed on the previous thread;
This survey is anonymous and will establish our weighted share BLT. The service is provided by KwikSurveys and there is no need to signup or input any personal details other than those specific to your BLT shares. I am unsure of what % we will need in order to be recognized by management, but I think its worth a shot. I have also added some additional questions to gain sentiment on a few topics
