I was thinking more along the line of the the article that arms posted a link to . . . which was about a maintainance of equipment failure, much of which is outsourced.

Then again myshiptracking was reporting Donghae Star had made its way to Koolan without leaving its previous port of call in Vietnam . . . which could be perceived as someone not dotting there i's and crossing their t's (not paying attention to detail).

There are also other scenarios in the link about the regulations and guidlines of loading iron ore fines onto ships . . . such as a ship not being made adequately prepared for its next load, engine vibrations, age and construction of the particular ship, etc.
