Don't worry about APRA they know their place, sit on the side lines and yell some encouragement when Joe blow gets tackled. They're just there to keep the public satisfied.
IMHO every level of CBA knew what was going on but it was worth running the risk of getting caught because the cost of implementing a fail proof system didn't make sense, this might seem crude to non business savvy lefties but it is why CBA is so successful. No employees were involved in the criminal activity, it was purely a business decision not to spend the money.
