yes providers are certainly ripping off as much as they can.
the 4 corners report a few weeks ago showed that the NDIA have a policy of "just paying" invoices without question, a policy established some years ago.... probably the ideological supporting private enterprise reasoning.
zero scrutiny of the actual cost of services rendered or cost of goods provided enables rip offs.
