Medgate are incentivised for their patients to remain supported end-to-end in a telemedicine consultation (also by the time to diagnose and accuracy). I don't believe there is another way to diagnose a respiratory patient via telehealth without DX. It is also faster and more accurate within a telehealth setting.

While switzerland is mandated to use private health insurers, there is a large % of the developed world that use private health insurance which are all focused on optimising the above metrics (costs & accuracy)
