Chronic Obstructive Pulmonary Disease (COPD), including emphysema and chronic bronchitis, is a major world health problem with significant morbidity, mortality and burden on health care. It may presently be underdiagnosed due to lack of investigative modalities that can detect early emphysema. On the basis of mathematical modelling and acoustic measurements on open cell foam analogues we predicted that sound would travel faster through emphysematous compared to normal lungs. Methods: 8 patients (average age 60.8 1.5 years, mean SEM, 3 males, 5 females) who had a single lung transplant for severe emphysema were studied. For each lung, pseudo-random noise was injected into the supraclavicular space and monitored by 7 sensors placed on the anterior and posterior aspect of the patients chest. A cross-correlation technique was used to estimate the time of first arrival from which sound velocity was determined. Results: Increases in sound velocity in the lower back sensors were consistently seen in the native emphysematous lung (154 9 m/s) compared to the collateral transplanted lung free from emphysema (75 6 m/s: p < 0.025, 1 tailed paired t-Test). Conclusion: Sound velocity is a useful measure to differentiate between a healthy lung and a lung with severe emphysema. We plan to investigate the diagnostic utility of this novel method to detect early emphysema in future studies.
