It certainly does open up a can of worms. Responsibility can be spread accross the mining team, however the geologists would wear a significant proportion.

Perhaps they didn't drill enough before mining? Perhaps the intrep was incorrect to pit mapping? Perhaps the geos didn't have a good handle on where the gold was sitting? Perhaps the there wasn't stringent supervision of ore mining or stockpile managment? And a whole host of other possibilities...

This would have come out of the resource and reconciliation audit - would be an interesting read indeed...
