Whatever you do, just don't tick the single box within the text. If you do you forfeit your votes, regardless of how you fill out anything else on the form.
I am positive these forms are deliberately designed to get people to make mistakes where they think they have voted for themselves, but have actually given their vote to the discretion of the chairman.
