Task 2. What are BLTs Objective and Fact Based Failures?

Task 2 - We need to ensure consistency in the nomination of the failures overseen by the current Board of BLT. Please feel free to nominate any OBJECTIVE and FACT BASED failures you believe should be included as part of the lobbying of significant holders and in formal communications to BLT. To ensure this process remains as efficient as possible... please ensure posts to this thread are for unique "failures" (I.e. once a failure has been identified please don’t add to the same theme unless absolutely necessary).
